Registration Document Verification Requirements


DOCUMENTS REQUIRED AT REGISTRATION DOCUMENT CHECK:

According to the rules set forth by Little League International®, all players MUST attend a Document Verification event each year and provide the following:

  1. PROOF OF AGE 
    Original document (no photocopies), such as a birth certificate MUST be presented. Per Little League official rules, passports are NOT an acceptable form of age verification.

  2. PROOF OF RESIDENCY or completed SCHOOL ENROLLMENT FORM
    Each year, residence shall be established and supported by documents dated or in force between February 1, 2021 to February 1, 2022. One document is required from each of the three groups listed below.  There are no exceptions.  Must be ORIGINALS only.  No photocopies or online statements are accepted. 

    Note: WE STRONGLY SUGGEST THAT YOU USE THE SCHOOL ENROLLMENT FORM in lieu of the residency documents listed below.  You never need to verify residence for as long as your child is at the same school when you use this form.  If you do elect to use the School Enrollment Form, it MUST be signed by the Principal or Vice-Principal.
      

     
    GROUP I
     - Driver’s License (photo ID of parent(s) or guardian(s) with qualifying residence address)
     - School records (home address of player’s parents or guardians)
     - Vehicle records (i.e., registration, lease, etc.)
     - Employment records
     - Insurance documents (with residence address) 

     GROUP II
     - Welfare/child care records  
     - Federal records (i.e., Federal Tax, Social Security, etc.) 
     - State records 
     - Local (municipal) records - (HB Water/Sewer Bill)
     - Support payment records
     - Homeowner or tenant records 
     - Military records  

    GROUP III
    - Voter’s Registration 
    - Utility bills (i.e., - gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
    - Financial records (loan, credit, investments, etc.)
    - Medical records 
    - Internet, cable, or satellite television records 

  3. A Completed MEDICAL RELEASE FORM

  4. A signed LITTLE LEAGUE PARENT CODE OF CONDUCT FORM

Seaview Little League is an "ALL VOLUNTEER" organization and we require your involvement to function.

A member from each player's family MUST volunteer for an activity at Seaview.  Activities range from helping at Opening Day, Try-Outs, Snack Bar, Field Maintenance, Pancake Breakfast, etc.  In addition, each team needs volunteers for the following:  Manager, Coach, Team Parent, Umpire, Scorekeeper, Team Photographer, etc. 

For the protection and safety of our players, o
ne REQUIRED volunteer from every family MUST complete a volunteer application and pass a background screening.  The application is completed online during registration when you select a volunteer position.  By filling out your name and email on the volunteer form, Little League's approved background vendor will send you a personal link for you to complete the confidential online background request - at no cost to you.
